How do I book an appointment?
Send us an appointment request and we'll get back to you to confirm a time. Or call the line for the care you need: Dental 480-409-0011, Aesthetics & Skin 480-997-5452.
Is an appointment request a confirmed appointment?
Not yet. A request tells us what you're looking for and when; we confirm the actual time with you by phone or email.
Which number should I call?
Two lines, one building. Dental: 480-409-0011. Aesthetics & Skin: 480-997-5452.
What if I need to cancel or reschedule?
Call the line you booked with as early as you can. Appointments cancelled after the cancellation deadline are charged a $100 fee.

What are your hours?
Monday to Wednesday 8am to 5pm, Thursday 7am to 3pm, and Friday by appointment.
Do you see children and families?
Yes. Dr. Blyth provides general and family dentistry, so the whole family can be seen in one place.

What happens at a first dental visit?
We go over your history, Dr. Blyth examines your teeth and gums and takes any X-rays he needs, and you leave with a clear plan before anything is scheduled.

Do you offer clear aligners?
Yes. Dr. Blyth offers orthodontic treatment with Invisalign clear aligners.

Who performs injectable treatments?
Ashli Blyth, RN, our aesthetic nurse injector, performs wrinkle-relaxing injections, fillers and collagen stimulators.

Who does facials and peels?
Kandace Schultz, a licensed esthetician, plans and performs facials, chemical peels and skin treatments.

Who provides wellness care?
Kristina McKinzie, FNP-C, a nurse practitioner, provides medical weight loss, hormone replacement therapy and peptide therapy.

Memberships
What memberships do you offer?
Two plans: Girl Math, and Health & Wellness. Ask us about either and we'll walk you through the details.
How do I cancel a membership?
Membership terms, including how to cancel, are set out in the membership agreement you sign when you join. Ask us about membership and we'll go through them with you. This is separate from our appointment cancellation policy.
Gift cards
Can a gift card be used for any service?
Yes. Gift cards are valid for any service, on the Dental side and the Aesthetics & Skin side.
Where do I buy a gift card?
Online, any time, through our gift card page. Purchases are handled by Boulevard, our booking and payment provider.
